In-Clinic Health Information Policy When you receive health services of any kind in-person at our clinic, we collect individually identifying health information from you. This information is shared securely within the clinic and with other health service providers strictly as needed to provide you with care.

The health information you provide to us in-person is collected, used, and disclosed in accordance with the provisions of the Alberta Health Information Act (HIA). It is primarily used to:

• Provide diagnostic, treatment, and care services to you (Section 27(1)(a)).
• Bill the Alberta Health Care Insurance Plan or your private insurance carrier for services provided (Section 27(1)(b)(g)).

The privacy provisions of this legislation require that we protect your health information from unauthorized access, use, disclosure, or destruction.
